What is Home Health Care?

Home Health Care includes a wide range of healthcare and social services that are provided in the comfort of your home. 
Services may be provided in other settings, such as an assisted living facility. Home Health Care is often provided following
a hospital stay to help speed healing and recovery.

 

How do I qualify to receive these services at home?

1.       Your Physician must sign a plan of care that specifies services.

2.       You must be homebound; absences from home must be infrequent, of short duration, or to get medical care. Doing so takes considerable and taxing effort.

3.       You must require intermittent nursing, physical therapy or speech therapy.

 

Who determines what services I will receive?

Your physician determines what is medically necessary and appropriate and must provide orders. Your insurance 
determines your coverage to pay for these services.

 

Is there a cost?

This depends on the service you receive and your benefits. Medicare covers 100% for services if deemed medically 
necessary. You may be responsible for a co-pay, co-insurance, or other expense. This will be explained to you once 
your benefits have been verified before you leave the hospital.
